A Future Living experienced and independent Support Coordinator will bring your NDIS Plan to life by arranging the services you need to get the most out of what you have been funded for. This might include:
- Independent/Supported Living
- Aides & Equipment to live independently
- Access to your local community
- Mainstream support connections
- Building personal capacity
- Liaison with your Plan Manager
- Specialised Allied Health Assistance
- Social and recreational opportunities
Once you have engaged Future Living as a Support Coordinator, we will:
- Get to know you, your goals, and your Plan
- Identify the right Service Providers to support you
- Negotiate contracts and establish Service Agreements
- Organise any capacity or functional assessments as may be needed
- Troubleshoot along the way
- Support you in your NDIS Plan Review.
What is a Support Coordinator?
Support Coordination is a NDIS funded support for people who need extra help implementing their plan. This could be because your plan is more complex and you require more support to get what you need, or there may be specific identified goals such as accommodation exploration that a Support Coordinator can assist you with.
A Support Coordinator will negotiate and coordinate services from a range of known suppliers or disability Providers and address any service delivery issues that may arise. They can help build your capacity to understand and navigate the NDIS. A Support Coordinator will also help strengthen your ability to connect with broader systems of supports as well as optimise your informal networks.
At Future Living, we offer two different services: Support Connection (Level 1) and Coordination of Supports (Level 2) to help you engage with your NDIS plan and Service Providers.
